Rather than advocating a maximalist - eliminate the FDA! - position, why not start with a call for serious reform to better align the FDA's goals, risk preferences, and incentives."

If people are too stupid to make medical decisions for themselves, then they're too stupid to make medical decisions for everyone else.
And if they're smart enough to pick politicians to make medical decisions on their behalf, then they're smart enough to pick medical experts to advise them for their own medical decisions.

Because I think institutions, once established, work for the own benefit, regardless of the harm they may be causing to the general public.
It's why the War on Drugs still lumbers on, causing 1.4 million people to be kidnapped and caged every year on drug charges.
Police/prison guard unions, rehab centers, and drug companies vigorously fight decriminalization, because ending the War on Drugs would reduce their income and power.
It's why the wars in Iraq / Afghanistan continue, 20 years after they started, to no discernible benefit to the US general public. That's because military contractors, government employees, and politicians benefit from the trillions of dollars of war spending that they now control.
IMO, without the right of exit, all institutions turn into parasites.
