My Dad had to make that decision when my Mom had me.  It was a long, difficult birth, and the doctors told him that they might have to cut me up in order to get me out before she died.  Thankfully, they were eventually able to get me out alive, albeit heavily bruised and with a head shaped like a cone.  (For the record, he chose my Mom).
