Robb Devoe Malpractice insurance is part of it.  But mandatory FDA testing, Certificate of Need laws, patents, medical licensure, immigration restrictions, Obamacare, Medicare/Medicaid, DEA regulations, cross-state insurance regulations--there are lots of factors. .
