"but under no circumstances should I be hired by an actual hospital. " Why shouldn't a hospital be able to hire non-government approved surgeons? Consider.  Imagine that you're a PhD scientists who has invented a new treatment for aneurysms.  You've done the procedure dozens of times on monkeys.  Who would you rather have do the surgery?  A neurosurgeon who has never done it before?  Or the inventor of the procedure? Or imagine you need a common procedure.  Who would you rather have perform it on you, a nurse who has apprenticed assisting with hundreds of surgeries, or a neurosurgeon fresh out of med school?
